Vacancy Name, Educational Qualification :-

The post to which the successful job seekers applying in this office will be appointed is Honorary Health Worker. To apply for this post you must have passed minimum secondary or equivalent examination from any government recognized board.

Application Procedure:- To apply for this post you must apply through offline as there is no online application form. The steps you need to follow for offline application are-

1) First download the official notification given below this notification.

2) Then you will see an application form below that notification take a print out of it on a white A4 size paper.

3) Then write your own name, father’s name, mother’s name, guardian’s name, educational qualification, gender, age, address, Aadhaar number, a valid email id and phone number etc. and fill the form.

4) Then affix a passport size photograph of yourself in the specified space of the form and give a signature in the signature space.

5) Finally, attach one copy of all your required documents along with the xerox application form and send it in an envelope by speed post to the designated place within the specified time.

Necessary Documents:- Necessary documents to be submitted along with the application form are-

1) Self attested xerox of secondary school admit card as age proof.

2) Xerox self attestation of voter and Aadhaar card.

3) Xerox self attestation of all educational qualification documents.

4) A permanent resident certificate certified by the chairman of the municipality where you are a permanent resident.

5) Two copies of passport size photo self attested.

Selection Procedure:- Those who apply here will be interviewed based on the marks obtained in Madhyamik and 10 marks. After checking everything those who clear this interview will be directly recruited for the job.

Salary: All the job seekers who get job here will be paid Rs.4,500/- per month.
